OPIOID Pharmacology : A Review

چکیده
Pain is an unpleasant sensation that originates from ongoing or impending tissue damage. Management of different types of pain (acute, postoperative, inflammatory, neuropathic cancer) is challenging and yet the most frequent issue encountered by clinicians. Pharmacological therapy is the first line of approach for the treatment of pain and opioid drugs are prescribed for acute and chronic pain of moderate/severe intensity arising from malignant and nonmalignant diseases. The opium poppy was cultivated as early as 3400BC in Mesopotamia. The term opium refers to a mixture of alkaloids from the poppy seed. Opiates are naturally occurring alkaloids such as morphine or codeine and opioid is the term used broadly to describe all compounds that work at the opioid receptors. The physiologic modulation of noxious stimuli involves a highly complex system that integrates the actions of multiple opioid receptors and endogenous opioid peptides. Opioids produce their actions at a cellular level by activating opioid receptors. These receptors are distributed throughout the central nervous system (CNS) with high concentrations in the nuclei of tractus solitarius, peri-aqueductal grey area (PAG), cerebral cortex, thalamus and substantia gelatinosa (SG) of the spinal cord. They have also been found on peripheral afferent nerve terminals and many other organs. The efficacy of centrally applied opioids is well recognized, but when applied peripherally, for example in posttraumatic and inflammatory states, their actions are less reliable. Although they are associated with addiction, dependence, tolerance and abuse liability even then their place in pain management remains undebatable and unchallenged.

Opioid receptors: Structural and mechanistic insights into pharmacology and signaling.
Opioid receptors are important drug targets for pain management, addiction, and mood disorders. Although substantial research on these important subtypes of G protein-coupled receptors has been conducted over the past two decades to discover ligands with higher specificity and diminished side effects, currently used opioid therapeutics remain suboptimal. Luckily, recent advances in structural b...
متن کاملTreatment of opioid dependence with buprenorphine: current update
Opioid maintenance treatment is the first-line approach in opioid dependence. Both the full opioid agonist methadone (MET) and the partial agonist buprenorphine (BUP) are licensed for the treatment of opioid dependence. BUP differs significantly from MET in its pharmacology, side effects, and safety issues. For example, the risk of respiratory depression is lower than with MET. The risk of dive...
متن کاملInfectious Disease (ID) Learning Unit: What the ID Clinician Needs to Know About Buprenorphine Treatment for Opioid Use Disorder
In the context of an escalating opioid epidemic, infectious disease clinicians increasingly treat the infectious complications of injection drug use. In this learning unit, we review the history, pharmacology, and clinical use of buprenorphine as maintenance therapy for opioid use disorder, and we describe the process by which clinicians can obtain a buprenorphine waiver.
متن کاملUpdate on Opioid and Analgesic Pharmacology
One of the most remarkable advances in the past decade has been in unraveling the molecular processes in opioid pharmacology. There have also been substantial advances in our understanding of the mechanisms involved in the processing of nociceptive information. It has become obvious that this is a very complex process, involving multiple neuropeptides, neurotransmitters, and their respective re...
متن کاملNociceptin/Orphanin FQ Receptor Structure, Signaling, Ligands, Functions, and Interactions with Opioid Systems.
The NOP receptor (nociceptin/orphanin FQ opioid peptide receptor) is the most recently discovered member of the opioid receptor family and, together with its endogenous ligand, N/OFQ, make up the fourth members of the opioid receptor and opioid peptide family. Because of its more recent discovery, an understanding of the cellular and behavioral actions induced by NOP receptor activation are les...
متن کاملCurrent Topics Salvinorin A: A novel and highly selective n-opioid receptor agonist
n-opioid receptors (KORs) represent the principal site of action of dynorphin and related neuropeptides. Recently, Salvinorin A—a naturally occurring neoclerodane diterpene hallucinogen was identified to be a highly selective KOR agonist. In this brief review we summarize the known chemistry, pharmacology and biology of salvinorin A. Because salvinorin A profoundly alters human consciousness an...
